IMMUNIZATIONS
If your student received immunizations over the summer, please bring a copy to Mrs. McDonald, our school nurse, before 8/16/19. If you were sent a letter in May about your student needing immunizations and they have not been obtained, your student will not be able to start school until proof of immunizations are received.

MEDICATION
All non-prescription medications must be brought to the Cy Creek clinic by a parent/ guardian, and signed in. These must be in new, unopened containers. Students are not allowed to bring medications to the clinic.
If your student takes prescription medications and they will be in the clinic, administered by the nurse, they must also be brought by a parent/guardian and signed in. Please make sure all prescription bottles are new. Old containers will not be accepted.

GENERAL INFORMATION
Students requesting to go home for medical reasons need to call their parents from the clinic only. Students must be picked up from the clinic if going home for medical issues. Students will not be released from the clinic to drive home. If your student is ill and drives to school, the attendance office must be notified by the parent.
District Policy states that students are not allowed to carry or bring medication to school. All medication must be brought in the original container to the clinic by a parent or adult. Parents must fill out a permission form to administer medication in the clinic.
Antibiotics and similar medications ordered 1-3 times daily can usually be given at home. Medications given 4 times a day can be given at lunch if original container is received in the clinic and a parent permission form is filled out.
Inhalers and Epi-pens are allowed to be carried by students only after the Physician self-administration medication form has been turned in to the nurse. A new Physician form must be turned in to the clinic at the beginning of each school year.
Due to important changes in the Texas Administrative Code Chapter 97 rule 97.7 regarding control of communicable diseases, Cypress Fairbanks ISD has implemented the following changes in our exclusion policy in our efforts to preserve student health. Students who have a temperature at or above 100.0°F must be excluded from school until fever free for 24 hours without the use of a fever suppressing medication. Students with diarrhea must be excluded from school until diarrhea free for 24 hours without the use of a diarrhea suppressing medication.
